To allow children access to online services a parent will need to request proxy access.
To enable this to be granted we will need a copy of the child’s birth certificate with the requesting parent named on the certificate.

Despite the recent Coronavirus pandemic, it is still important your child continues to receive their routine immunisations and you are given the opportunity to discuss any concerns you may have about your baby/child. You can arrange an appointment with one of the practice nurses by contacting the surgery
Vaccinating your child protects them from serious illnesses
It is important that you register your baby at the practice at your earliest convenience. To do this you will need to complete a registration form at the practice. Please ensure you have the baby’s NHS number when completing the form, this can be found in the baby’s red book.
Please note the nurse giving the immunisations will require parental consent, if neither parents are attending please provide written consent.
The removal of moles, cysts, skin tags and ingrowing toe nails can be performed at the surgery. We also offer joint injections for certain musculoskeletal problems.
Please note you may be added to a short waiting list first before being allocated an appointment
It is strongly advised that women should attend the GP practice to have this test performed. Women aged 25-49 will be recalled every 3 years and for women aged 50-64 will be recalled every 5 years.
You can arrange and appointment with one of the practice nurses by contacting the reception team. However we do have a recall system which will prompt the practice to contact patient if the test is over due.
