New Patients Registrations
You will need to press 2 when you get into the lift at the East Leeds Enhanced Primary Care Centre as we are on the second floor!
To register as a new Patient you will need to complete registration forms available from Reception and provide 2 satisfactory forms of identification.
You will then need to have a “new patient check” with a Practice Nurse before your registration is complete. The purpose of this check is to obtain any medical history and to make us aware of any problems that need specialist attention.
If you need language support / translation services please let us know. Medical treatment is available from the date of registration. Please contact reception for further information.
